This event will focus on how the third sector helps to prevent unnecessary and inappropriate hospital admissions and alleviates delayed transfers of care.

Speakers include key stakeholders from the third and statutory sectors. The event focuses on services in Cardiff and the Vale.

 

When? Tuesday 6 March 2012 from 9.30am to 3.00pm

Where? Park Inn Hotel, Mary Ann Street, Cardiff

Lunch will be provided.


This event will help to:
  • Identify the third sector's contribution to preventing unscheduled admissions and alleviating delayed transfers of care.
  • Highlight the third sector services which promote independence and support people in their communities.
  • Explore current, new and different ways for the third sector and statutory sectors to work together in partnership.

 

Speakers from both the third and statutory sectors will give presentations during the day on:  

·         UHB work on preventing unnecessary hospital admissions and alleviating DTOC

·         Frail Older People’s Service Delivery Steering Group priorities

·         Locality-based working

·         Changes to the care sector

·         Working with Cardiff and Vale of Glamorgan Community Health Council
